      <image:caption>Zachary Goldfarb, BS, EMT-P, CHSP, CHEP, CEM®, is the principal of Incident Management Solutions, Inc., a New York-based certified emergency management consultancy specializing in planning, training, and operational leadership for emergencies and major events.  Clients range from the 1994 Woodstock II concert festival in upstate New York, where he served as the medical incident commander, to many hospitals and health care systems in the New York metropolitan area and across the country, where he has developed Joint Commission-compliant emergency management plans and implemented the Hospital Incident Command System (HICS).  Recent activities include developing a New York State Department of Health Comprehensive Emergency Management Program for all hospitals in New York State, and a set of healthcare facility evacuation plans, tools, and templates for all flood risk healthcare facilities in the State, revising the Airport Emergency Response Plan for Los Angeles International Airport, developing a standardized emergency management program for the nation’s 18 end-stage renal disease networks, and leading a series of Long-Term Care Emergency Management, Exercise, Technical Assistance, and Continuity of Operations Programs for the New York City Department of Health and Mental Hygiene, improving disaster readiness for the City’s skilled nursing, assisted living, and adult care facilities, home care and hospice agencies, and primary care centers. Validated by the United States Department of Defense as Medical Nuclear, Biological, and Chemical Subject Matter Expert Instructor, Chief Goldfarb was a consultant member of the federal Domestic Preparedness Training Team, participating in the nationwide instructor-training program preparing first responders in the 120 largest cities to manage the medical consequences of terrorism.  In 2006, he served on the Secondary Review Group of the national HICS-IV development project.  In 2009, he was in the first cohort to receive the prestigious new credential, Certified Healthcare Emergency Professional (master’s level) from the Board of Certified Hazard Control Management. Certified as a paramedic since 1978, Chief Goldfarb has served in many capacities during his 25-year career with the City of New York Fire Department’s (FDNY) Emergency Medical Service Command (and the New York City Emergency Medical Service, prior to their 1996 merger), including Deputy Chief, Bronx Borough Commander, Deputy Chief of Operations, Emergency Management Coordinator, and commanding officer of the Special Operations Division. He is the author of the New York City Emergency Medical Action (disaster) Plan, which promulgated one of the most frequently used medical incident command systems in the nation.  During both the 1993 bombing and the 2001 terrorist attacks on the World Trade Center, he served as an EMS commander at the scene.  In the aftermath of Hurricane Katrina, Chief Goldfarb led the Louisiana Special Needs Assessment of Katrina Evacuees (SNAKE) team for the National Organization on Disability (NOD), which observed, documented, and reported on the plight of special needs evacuees in the days and weeks following the storm. Zachary Goldfarb has an associate degree in paramedic science, a bachelor’s degree in public administration and management from the City University of New York and is a graduate of the New York City Leadership Institute.  A nationally certified emergency manager, certified healthcare emergency professional, certified healthcare safety professional (both Master’s level), and certified fire department incident safety officer (ISO) and health and safety officer (HSO), he is a member of the National Fire Protection Association’s Technical Committee on Health Care Emergency Management and Security, and lectures and writes widely on emergency management, incident command, and related topics. Chief Goldfarb was the Technical Editor of FireEMS Magazine and a member of the editorial advisory board of both Fire Engineering Magazine and the Fire Department Instructors’ Conference (FDIC) and is an advisory board member of Opus Communications’ Briefings on Hospital Safety.  He is also the Vice President and an active FireMedic with a volunteer fire department in Nassau County, New York; Chair of the Nassau County Fire Commission's Emergency Management Committee; and a command staff member of the Nassau County Medical Reserve Corps.</image:caption>

Prior to joining the firm, he was the Associate Executive Director of Elmhurst Hospital Center, a 525-bed municipal 9-1-1 receiving hospital and Level One Trauma Center serving western Queens, where his responsibilities included hospital emergency management, hospital police, and the support services division. Certified in Homeland Security, Robert M. Becker has had more than 40 years of progressively responsible background in leading healthcare and emergency service organizations during his career with the City of New York Health and Hospitals Corporation (HHC), the largest municipal hospital system in the United States.  He has served in many capacities, including 18 years as a senior hospital executive and 19 years leading the New York City Emergency Medical Service (NYC*EMS). At Elmhurst Hospital Center, Robert Becker was the principal liaison with governmental public safety and emergency management agencies at the federal, state, and local levels.  As the hospital’s emergency management coordinator, he chaired the Emergency Management Committee that governed the hospital’s disaster preparedness program in compliance with federal, state, and municipal codes and requirements.  In addition, he was the senior executive for a Hospital Police force of 60 Peace Officers, including a Detective Unit and a Technical Unit.  Mr. Becker was responsible for overall security of the two-million square foot, multi-building high-rise municipal hospital campus, including the safety of thousands of staff members, patients, and visitors each day. Chief Becker’s distinguished public safety career began in 1971, when he joined the ranks of NYC*EMS.  As an emergency medical technician, he served in a variety of assignments in the Field Services and Communications Divisions, before rapidly rising through the ranks of Lieutenant, Captain, Assistant Chief (Bronx, Brooklyn, and Staten Island Command), Chief of Field Services, and Chief of Operations.  He was the youngest person in New York City history to hold the “four-star” position of Chief of Operations. As the highest-ranking uniformed officer, his hallmark accomplishment was the reorganization of the agency, unifying all the uniformed divisions and special commands within NYC*EMS.  These divisions included Field Services, Communications, Support Services, and Special Operations; protecting the health of eight million New Yorkers daily with a diverse city-wide force of 1,900 uniformed personnel, 230 ambulance units, and an annual operating budget of $60 million.  During Chief Becker’s tenure, in 1982, NYC*EMS became the first city agency to adopt and implement the new Incident Command System, destined to become the national standard. During his career, Chief Becker received numerous honors and awards, including the EMS Medal of Honor and the Executive Director’s Medal, as well as several Pre-hospital Save Medals.  He is certified in homeland security (CHS Level IV) by the American College of Forensic Examiners Institute, and is a member of the National Fire Protection Association’s Technical Committee on Health Care Emergency Management and Security (NFPA 99-12).</image:caption>

An associate with the practice, Captain/Paramedic Cook brings a wealth of leadership, public safety, and emergency management experience to our clients’ benefit every day.  Louis recently retired as the Commanding Officer of FDNY EMS Special Operations HAZ TAC Battalion, and as a member of the FDNY Center for Terrorism and Disaster Preparedness exercise design team as a FEMA/EMI trained Master Exercise Practitioner. He is a highly decorated 28-year veteran of the New York City Fire Department EMS Command, and a 2001 World Trade Center attack survivor. His involvement with disaster medicine began with him serving as a Lieutenant in the FDNY Special Operations Command – Haz-Tac Battalion and member of the National Disaster Medical System - Disaster Medical Assistance Team (NY4) as an Emergency Medical Specialist.  During his FDNY career, he served as a Course Coordinator for EMS First Line Supervisors (Lieutenants) and Captains Training programs as well as teaching NIMS Incident Command System and Mass Casualty Event Management Strategies for Executive Level and Senior Command Staff Officers, and Medical Management of First Responder Mayday Situations. He also teaches Medical Planning and Support of Dignitary Protection and National Special Security Events. Captain Cook has also been an invited lecturer for the Ana Maria College Emergency Management programs. He also has experience in Medical Countermeasure Program planning, management and exercises. Additional instructor qualifications include: EMI Trained NIMS/ICS Instructor, DHS/FEMA WMD, AAOS Trauma Care, WMD/CBRNE-Radiological- Nuclear Responder, Incident Response to Terrorist Bombings, Biological Weapons Responder Awareness, and WMD Standardized Awareness. He is a FEMA certified Instructor/Trainer. He has also served as a Lead Instructor for FDNY's Rescue Medic program, EMS Hazardous Materials First Response Operations and Technician level programs. He possesses Diver Medical Technician, Technician Level Certifications in Rope Rescue, Extrication, Trench Rescue, Confined Space, Structural Collapse, Hazardous Materials Technician Level II and Draeger Rebreather SCBA Operation qualifications. Louis is a lecturer and frequent contributor to the Journal of Emergency Medical Services and has published articles in Critical Care Medicine and Fire Engineering.
